The Leadership and Management Questionnaire measures the perceptions about how often the Assessee exhibits the skills and behaviors ("competencies") that are critical to his/her work and to the organization's success.

For each question, please rate “how frequently does the Assessee engage in the behavior described?”

Rating scale 

1 = Never
2 = Rarely
3 = Sometimes
4 = Usually 
5 = Always

Be realistic, honest and accurate about the extent to which the Assessee actually engages in the behavior. 

DO NOT answer in terms of how you would like he/she to behave or in terms of you think he/she should behave.

DO answer in terms of how the Assessee typically behaves on most days, on most projects and with most people.

Be thoughtful about your responses. For example giving the Assessee "5s" on all items is most likely not an accurate description of his/her behavior. Similarly, giving him/her all "1s" or "2s"  or "3s" is not accurate either.
Most of people do some things more or less often than they do other things.

If you feel that a statement does not apply to the Assessee, it’s probably because he/she doesn’t frequently engage in the behavior. In that case, assign a rating of 1.
