this is the registration form to attend the MLS Actions Taken Review virtual workshop "Monitoring Covid-19 pandemic: epidemiological and sequencing data for decision-making", which will take place online on the 16th of December 2021, from 11.00 to 13.30 (CET). Additional information regarding the event and the link to attend will be sent in due time to the email address you will provide in this questionnaire for registration.

Please note that the event will be recorded and shared on MediLabSecure's website. Therefore, we will consider that anyone who will ask any question or comment during the meeting agrees and gives consent to be recorded, and the video to be shared on the website. If you would like to ask any question or comment during the presentation without being recorded, please type your question in the Chatbox and the moderators will address it to the speakers.
